Reason For Report:

6 rounds until map change, at the beginning of the round, he said "I'm gonna kill you, Potato" and then RDMed me. I don't have any evidence of the first one, but I'm sure the logs will show him shooting me 2 seconds into the round. That was my T round.

After that round, I asked him why he did it. He didn't give a serious response, and instead said John Cena. I warned him that I would report him on the forums if he did it again.

2 rounds until map change, about 30 seconds into the round, I entered the room underground that has the T room Door in it. He followed me in, and I simply looked at him until he left so I could enter the T room. But instead, he immediately said KOS Potato, and killed me again, on my 2nd T round on that map.​

Given there is no evidence supporting the other deaths, he will only be punished for one, as our old logs are currently out of working order.
