I don't know what happened to your adminstats, but @CDriscoll banned you for 8 weeks and you got un-banned somehow, so we will let him discuss what happened there.

I've looked over this with BmTron, and this seems to be another case of globalized bans randomly ending early. His ban for the fourth offense is still valid, and was not appealed or voided. The current ban is, and should be, his fifth offense.

as you can see you were cooking the incendiary just because you were planning to throw it in to the t room when it opened but it backfired and killed a D.
